GENERAL PRACTICE SUPPORT

The West Vic Division provides support and information to nurses working in general practice through the provision of information, advice, education and networking approaches.

ACCREDITATION

The Practice Support Officer provides advice and resources in line with the third edition of the RACGP Standards for general practice to promote best practice models and support practices to obtain and maintain accreditation.

CHRONIC DISEASE MANAGEMENT

Chronic Disease Management (CDM) in general practice involves appropriate prevention, early identification and best practice management strategies.

Proper management of chronic disease entitles your practice to incentive payments.

EDUCATION COORDINATION

The practice support officer coordinates relevant professional development opportunities for all practice staff, including practice nurses, practice managers and administration staff.

IMMUNISATION

Information is sent to practices with up to date immunisation news and articles. The division also provides distribution of data loggers and cold chain support and can utilise nurse immuniser skills where necessary.

PRACTICE MANAGEMENT

The Practice Support Officer provides resources and coordinates Practice Manager network meetings.

PRACTICE NURSING

Nursing in general practice is one of the fastest growing areas of nursing in Australia. Practice nurses are now key members of the health team in almost all general practices and the role is only set to increase.
