QUALITY USE OF MEDICINES

Home Medicines Review (HMR)

The Home Medicines Review is a pharmacist sector funded program, provided by the Pharmacy Guild of Australia. It aims to provide a consumer-focused service to optimise quality use of medicines and increase consumer understanding of their medicines.

HMR involves the consumer, their general practitioner, and their pharmacy, and may be requested by other health care providers. A consumer can receive a visit from a pharmacist to learn more about their medicines, have a comprehensive education about how best to use their medicines, and have a written report sent back to their referring GP. The program aims to optimise benefit and minimise harm from medicines.

National Prescribing Service

National Prescribing Service LTD (NPS) is the Quality Use of Medicines (QUM) service agency for Australia's National Medicines Policy. NPD provides a reliable source of independent therapeutic information on medicines. Quality use of medicines involves selecting management options wisely, choosing suitable medicines if a medicine is considered necessary; and using medicines safely and effectively. NPS aims to achieve improvements in health through better use of medicines, helps develop knowledge on QUM, and to provide innovative, relevant QUM services for health professionals and for consumers. NPS is funded by the Australian Government of Health and Ageing.

West Vic Division of General Practice provides NPS services by employing a trained NPS facilitator to deliver the QUM program. Practice visits for GPs and practice nurses and educational events for pharmacists are provided, based on QUM resources from NPS. Workshops, newsletters, clinical audits and case studies are provided by the program. When participating, GPs have the opportunity to reflect on their own practice and explore and apply best practice therapeutic guidelines in their prescribing. The facilitator provides QUM presentations to consumer groups, promoting messages about using medicines wisely, effectively and carefully.
