cdmNet (Chronic Disease Management Network)

The Division is supporting the uptake of cdmNet, a service provided by Precedence Healthcare.

The service permits GPs and their nurses to create GPMPs and TCAs which:

• are based on RACGP best practice guidelines are based on individual patient's history and measurements can be modified by doctor or nurse can be managed and reviewed at any time permit allied health professionals in the care team to collaborate on-line
